Ines took the flat-number line off the shop's address form because almost nobody filled it in. Four thousand parcels ended up in one man's hallway in Flat 1.
The mistake
The address form drops its optional flat-number field because low fill rates made it look untidy in the UI, and the change ships straight to production without checking how the field is used downstream. The shipping/courier logic still resolves deliveries by street address alone, so without a flat number every parcel for a multi-unit building resolves to the same single unit, misrouting thousands of deliveries to one resident instead of failing or prompting for disambiguation.
