Ines swapped the refund and print buttons so the one everybody used got the big spot. Toby then refunded all forty-one orders instead of printing a single label.

The mistake

A staff tool's layout is changed so a frequently used button and a destructive one (Refund) swap places. Operators who click by position, not by label, now hit Refund on every order. Nothing asks for confirmation and there is no undo window, so each misclick goes straight through. Moving the buttons back to fix it changes the layout a second time, and the muscle memory just retrained on the new positions sends people to the wrong button again. Click counts gathered while a layout is moving around measure where people expected the button to be, not which action they actually wanted.
