The mistake
A developer needs a real address to test a transactional 'order cancelled' email, so he grabs the first name off the live customer list instead of using a seeded test account or sandbox address. The test send is then re-run in a loop with no send-count guard, rate limit, or idempotency check to stop it, so the loop keeps firing against that same real customer until it has gone out 412 times.
