Dale ordered a tidy-up that deleted every account nobody in the company had ever signed into. It found exactly one, and it was his.
The mistake
The cleanup job flags an account for deletion based purely on absence of login history, treating 'never signed in' as a proxy for 'unused and safe to remove.' That heuristic doesn't distinguish a genuinely abandoned account from one that's valid but simply never used interactively by its owner, so a live, correctly-provisioned account gets caught and deleted along with the dead ones.
