The mistake
The team implements a manual QA gate by rewriting the destination address on every shipping label to the office instead of adding a separate hold/inspection flag on the order. Because the change is applied to the label-generation step for the entire week's batch rather than a sample or a staging queue, all four thousand parcels are physically routed back to the office instead of to customers. There's no sampling or partial rollout, and the inspection step itself only reviews eleven of the four thousand parcels before being declared good enough.
